The Open Secure AI Alliance, a group hosted by the Linux Foundation whose backers include Nvidia, Cisco and CrowdStrike, published a proposed framework on 11 August called SAFE — Shared AI Findings Exchange — for reporting security incidents caused by AI agents. It covers agents gaining unauthorised access to third-party systems, disclosing confidential information, or continuing to probe a live target after the operator suspects something has gone wrong, and it covers near misses as well as actual harm. Members would preserve prompts, agent traces, tool calls, identities, permissions and credentials as evidence; notify affected organisations as soon as possible and demonstrably exposed customers within 72 hours; file a confidential report within four business days and a public one, with an analysis of what failed, within 30 days. It is voluntary, and for now it is only a document: there is no way to submit a report and no implementation timeline, just a GitHub repository collecting feedback.

Why it mattersEvery other kind of security incident has a disclosure norm built up over decades. Agents have none, and the awkward case is specific: not that you were attacked, but that the software you deployed went and did something to someone else. Right now a company in that position has no expectation to report it and every reason not to. A shared format is the necessary first step, though a voluntary framework with no submission mechanism is also the easiest kind to announce. Watch whether anything gets built, and whether the first firm to use it is punished for the honesty.
